Job Summary
Recovery Facilitator at 4C Health delivers mental health recovery by evaluating clients, developing care plans, coordinating with treatment teams, providing recovery coaching and skills training in home or community settings. Responsibilities include completing client assessments and discharge-ready care plans, ensuring care-plan and data accuracy, generating progress reports for referrals, coordinating with external treatment partners, facilitating weekly case management and skills development, delivering services aligned with treatment plans, and participating in supervision and ongoing education. Qualifications include a minimum high school diploma or GED (associates/bachelor preferred), with potential additional certification for substance use disorder work; must pass background checks and drug screening. Required Qualifications
- High School graduate or GED
