Job Summary
Recovery Counselor responsible for assessing clients across personal, medical, emotional, social, and environmental domains to plan treatment through home, community, and office visits; provide motivational, client-centered, strengths-based, trauma-informed services to individuals with serious mental illness and co-occurring substance use disorders using harm reduction; conduct 75% community-based services; perform case management with shared caseload; document services per policy and state requirements; provide crisis/on-call coverage; coordinate recovery plans, MSR/MAR documentation, IM+CANS and LOCUS; assist with entitlements (SSI/SSDI/Medicare/Medicaid/LINK); accompany clients to appointments; advocate and empower clients; engage in team-based care and multi-disciplinary collaboration; participate in team meetings and electronic communications; perform other duties as assigned; evening/weekend availability and valid IL driver's license required.
Required Qualifications
- Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human services field
- Bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting
- Licensed Practical Nurse under the Nurse Practice Act [225 ILCS 65]
- Certificate of Psychiatric Rehabilitation from a DHS-approved program, a high school diploma/GED, and two years’ experience in a mental health setting
- CRSS certification through IAODAPCA
- Family Partnership Professional certificate from IAODAPCA in good standing
- Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with one-year experience in a mental health setting
- High school diploma or GED and five years of supervised clinical experience in a mental health setting
- Valid Illinois Driver’s License with daily access to a vehicle. Must have $100,000/$300,000 auto liability insurance.
