Job Summary
Complete assessments, recovery plans, and risk assessments while coordinating personalized treatment including clinical and psychosocial interventions. Work closely with prescribing colleagues, health services, and community partners to connect individuals with support that improves health, wellbeing, and independence. Respond to safeguarding concerns, make appropriate referrals, and promote harm reduction, naloxone, and peer support. Maintain accurate, timely, and confidential records throughout the treatment and recovery journey. Based in Harpurhey as part of the North Manchester team, you will support people from their first contact through to meaningful change.
Required Qualifications
- Right to work in the UK at the time of application
- Experience from drug and alcohol services or a related field
- Strong assessment skills
- Communication skills
- Case-management skills
- Organised
- Confident using IT systems
- Ability to balance independent working with being part of a supportive multidisciplinary team
- Belief in people and their ability to make positive changes
- Enhanced Disclosure and Barring Service (DBS) check
