Recovery Coach - Adults

Establish a collaborative, respectful relationship with adults receiving services and their families. Develop clinical assessments, service plans, and crisis safety plans based on individual strengths and needs. Coordinate and implement recovery goals while monitoring service receipt and response to treatment. Maintain complete clinical records documenting progress, services provided, and reasons for non-provision. Facilitate Child and Family or Adult Recovery Team meetings to assess ongoing needs. Obtain prior authorizations for inpatient, partial hospitalization, and residential services. Coordinate care across medical, behavioral health, probation, and crisis centers within a trauma-informed framework. Provide face-to-face, telephone, and collateral support including transportation to medically necessary services.
