Job Summary

Conduct outreach to clients to facilitate engagement in Medication-Assisted Treatment and mobile community services. Deliver health coaching and recovery support covering self-advocacy, wellness management, medication adherence, and daily living activities. Provide transportation and accompany clients to medical appointments, while coordinating care plans with providers to ensure goal-directed, collaborative transitions. Act as a liaison between the Agency and community resources including courts, hospitals, and probation services to facilitate referrals. Maintain accurate documentation of contacts and service delivery, monitor client progress, and provide crisis intervention and follow-up. Represent the Agency professionally at community events and promote drug and alcohol prevention services through education and training. Drive company vehicles as needed and report directly to the director.

Required Qualifications

  • High School Diploma or GED
  • Provisional certification (CRPA-P) upon hire and receipt of full certification (CRPA) within one year
  • Knowledge of medication assisted treatment
  • Opiate overdose prevention knowledge
  • Substance use disorders treatment knowledge
  • Recovery services knowledge
  • Relevant life experience: personal experience living with a disability
  • Relevant life experience: history of substance use
  • Relevant life experience: social/emotional challenge
  • Willingness to draw upon life experience to serve as a role model to support the recovery goals of individuals served
  • Knowledge of community resources
  • Community outreach knowledge
  • Ability to cultivate a culture of inclusion for all employees
  • Adherence to all safety protocols and procedures
  • Health and safety awareness
