Job Summary
Reassessor position completing in-home Medicaid Reassessments to determine continued eligibility and level of care for participants receiving in-home and community-based services. Responsibilities include evaluating medical, functional, and social needs; documenting assessments clearly in designated software; verifying care plan tasks; communicating with providers, family members, and informal supports; maintaining knowledge of regulations; participating in quality assurance; and upholding confidentiality and ethical standards. Requires independent work, strong organizational and communication skills, proficiency with electronic case management systems, and a valid driver’s license for home visits. Bachelor’s degree and Medicaid Reassessment Certification are required; prior experience with assessments or case management in social services or healthcare is preferred.

- Bachelor’s degree in any field required
- Strong assessment, interviewing, and documentation skills
- Ability to manage time efficiently and meet strict deadlines
- Excellent written and verbal communication
- Proficiency with computers, electronic case management systems, and Microsoft Office applications
- Strong interpersonal skills and the ability to work independently and as part of a team
- Must successfully complete Medicaid Reassessment Certification
- Must possess a valid driver’s license and reliable transportation for home visits
- Must pass all required background screenings
