REACH Mobile Crisis Coordinator

Job Summary

Required Qualifications

• •Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QDDP)
• •Minimum of a bachelor's degree in human services or related field from a USDOE or CHEA accredited institution
• •No less than one year of fulltime relevant experience
• •Current, unrestricted Virginia license as an LPC, LCSW, or LMFT or closely related
• •Must be registered (or eligible to register) as a QMHP-T with the Virginia Board of Counseling or hold a valid Licensed Mental Health Professional-Resident or Supervisee
• •Those not currently registered must be eligible to apply and must submit their QMHP-T application upon hire, with registration obtained within 30 days, unless they are a Licensed Mental Health Professional
• •Must have a valid, unrestricted, state appropriate driver's license and vehicle insurance
• •Must be physically able to lift a minimum of 50 pounds, bend and assist individuals with mobility needs
• •Must be able to hear with in normal ranges
• •Must be able to talk in order to articulate crisis prevention, intervention and de-escalation supports
• •Must be able to see with or without accommodation to observe individuals
• •Must possess the necessary abilities to perform tasks such as develop written communication as well as reviewing work to determine accuracy
• •Ability to communicate effectively with internal and external clients
• •Must demonstrate capacity to effectively communicate verbally and in written form

• •Must be able to perform each essential duty satisfactorily
• •Must be able to remain in a stationary position for extended periods of time
• •Must be available for weekend shifts
• •Must be available for 12 hour shifts 9P-9A
• •Must be available for a 4 hour shift
• •Must be able to travel which may include exposure to inclement weather
• •Must be able to complete required training within prescribed timeframes
• •Must be able to comply with all ESPH policies and procedures, HIPAA guidelines, DBHDS rules, Medicaid/Medicare federal guidelines
• •Must be able to attend all staff and program meetings as requested
• •Must be able to maintain regular attendance and punctuality
• •Must be able to manage use of PTO time (if applicable)
• •Must be able to follow all policies and procedures regarding time off
• •Must be able to complete required training within prescribed timeframes
• •Must be able to perform related duties as requested
• •Must be able to provide rapid response to individuals experiencing a behavioral health crisis
• •Must be able to meet the individual in an environment where they are comfortable to facilitate service engagement, stabilization, and resolution of the crisis when possible
• •Must be able to provide appropriate care/support/supervision in order to maintain safety for the individual and others, including but not limited to crisis intervention, community stabilization, care coordination, and treatment planning
• •Must be able to refer and link to all medically necessary behavioral health services and supports
• •Must be able to coordinate with behavioral health providers providing services to the individual throughout the delivery of the service
• •Must be able to maintain linkages and relationships with community partners
• •Must be able to ensure the completion of all required documentation for Mobile Crisis services, including but not limited to assessment, treatment planning, and Crisis Education and Prevention Plans (CEPP)
• •Must be able to participate in required meetings with REACH Team Members and other applicable parties
• •Must be able to coordinate admissions/discharges and requests for appropriate individuals to/from REACH Crisis Therapeutic Home in collaboration with the REACH CTH Supervisor
• •Must be able to coordinate admissions/discharges and requests for appropriate individuals to/from REACH Mobile crisis supports in collaboration with REACH Team Lead
• •Must be able to monitor individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports in accordance with crisis guidelines and individual's plan
• •Must be able to plan, structure and prompt social and leisure time activities, including activities that build skills for behavior management, stress management, communication, adaptive living and daily living
• •Must be able to collect and enter required data and produce documentation on administrative and service delivery and overall utilization of REACH services within established timeframes and according to set protocols
• •Must be able to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc
• •Must be able to complete satisfactory background check/screening as referenced in Easterseals PORT Health Human Resources Policy A6
• •Must be able to complete satisfactory Tuberculosis (TB) Screening
• •Must be able to complete current satisfactory completion of therapeutic interventions training - either Therapeutic Options of Virginia (TOVA) or Crisis Prevention Institute (CPI) approved curricula
• •Must be able to obtain current approved certification to provide emergency CPR/First Aid
• •Must be able to complete Agency Orientation
• •Must be able to complete Confidentiality and HIPAA training
• •Must be able to complete Human Rights training
• •Must be able to complete Personnel and Benefits training
• •Must be able to complete Emergency Preparedness and Emergency Response training

Desired Qualifications

• •Graduation from an accredited college/university with a Master's Degree in Counseling, Social Work, Psychology, or closely related field preferred
• •Current, unrestricted Virginia license as an LPC, LCSW, or LMFT or closely related preferred
• •Those not currently registered must be eligible to apply and must submit their QMHP-T application upon hire, with registration obtained within 30 days, unless they are a Licensed Mental Health Professional
• •Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs preferred
• •Knowledge of the MH and I/DD systems of care
