Job Summary
The Re-entry Coordinator collaborates with detention and community partners to develop service and discharge plans for youth in detention, build connections with community-based providers to support reintegration, and engage youth to identify strengths, needs, and interests. Responsibilities include coordinating with DYFJ case management, programs, detention services staff, partner health services and DOE staff; ensuring access to mental health, medical, literacy, substance use, emotional regulation, life skills, prosocial development, recreation, and vocational and career support; conducting field visits to coordinate care, preparing and maintaining reports, and performing case management duties to promote successful return to the community. The role supports cross-agency planning with City agencies (including DOE and the Department of Correction) and fosters family engagement throughout the transition.
Required Qualifications
- A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.
