Job Summary
Provide direct service to assigned caseloads of individuals transitioning back into the community prison setting, including comprehensive assessment, service planning, and coordination of stabilization needs. Conduct outreach to clients, families, and other involved persons while assuring linkages to medical, financial, social, rehabilitative, addiction, and psychiatric services. Deliver crisis intervention, treatment, and support in the absence of viable alternatives, and perform recordkeeping, statistical tracking, and quality assurance activities. Collaborate effectively with parole and community partners to meet enrollment and completion benchmarks.
Required Qualifications
- Demonstrated ability to work independently and to initiate change
- Excellent skill and judgment in a range of clinical and case management activities
- Ability to collaborate effectively with a team and have superior interpersonal skills in dealing with other providers and client supports
- Excellent written and verbal skills
- Proficiency in the use of computer software such as MS Word, Excel, PowerPoint, Desktop Publishing, Outlook, etc.
- Excellent time management skills
- Associate or bachelor's degree
- At least one year of Case Management experience with a bachelor's degree OR two years' experience with an associate degree
- Valid Driver's License with a satisfactory driving record
- Personal vehicle for job requirements
Desired Qualifications
- Two years' experience working with individuals involved in the criminal justice system
