Job Summary
Assist vulnerable patients in locating and planning access to various healthcare-related services and resources within their community upon release from the correctional setting. Address questions or concerns patients with an upcoming release may have about navigating the healthcare system post-release. Requires an Associate's degree (Bachelor's preferred) in human services, one to two years of case management experience, and a background investigation with security clearance. Must maintain current tuberculosis documentation and active CPR certification. Full-time day shift position at the Northeast Correctional Center in Bowling Green, Missouri.
Required Qualifications
- Associate's degree in human services or related field
- Minimum of one – two years providing case management or a similar service to vulnerable populations/groups
- Must be able to pass a background investigation and obtain agency security clearance where applicable
- Must maintain current tuberculosis documentation
- Must maintain active CPR certification
- Full-Time Day shift
Desired Qualifications
- Bachelor's degree preferred
- Knowledge of Microsoft Office, specifically Microsoft Excel
- Prior experience working in a correctional healthcare program, criminal justice, or forensics
