Job Summary
Rapid Rehousing and Transitional Housing Case Manager responsible for intensive case management for youth (18-24) experiencing homelessness, developing individualized service plans, coordinating with landlords, and connecting clients with resources to secure and maintain housing. Key duties include intake and assessments for eligibility, ISP development and weekly updates, housing navigation, landlord mediation, referrals to employment/education/mental health/financial supports, conducting home visits, ensuring program compliance, documenting interactions in HMIS/EHR, facilitating life-skills workshops, and collaborating with team members and community partners to achieve housing stability. Requires strong knowledge of Housing First and trauma-informed care, advocacy, mediation, cultural competence, and the ability to manage a 15-client caseload; requires reliable transportation and Nevada driver’s license. Starting pay: $26.44 per hour with benefits including 403(b), health, dental, vision, EAP, PTO, holidays, and more.
Required Qualifications
- 2+ years of case management experience
- OR Associate’s degree in a related field with 2 years of case management experience
- Experience supporting individuals in transitional or rapid rehousing programs is strongly preferred
- Reliable Transportation: must have a reliable vehicle, a valid Nevada driver’s license, and updated insurance
- Caseload Management: must be able to manage a caseload of 15 clients
- Must be available for home visits and to see clients at the agency or in their homes
- Must be able to lift up to 15 pounds at a time
- Flexibility to work occasional evenings and weekends based on program needs
