Job Summary
Conduct comprehensive assessments of residents to identify medical, social, and psychological needs. Develop and implement individualized care plans addressing unique resident goals. Coordinate with healthcare providers, including physicians, therapists, and social workers, to ensure comprehensive care. Ensure seamless transitions between care settings, including hospital, rehabilitation, and long-term care. Monitor residents' progress and adjust care plans as necessary. Maintain accurate and detailed records of assessments, care plans, and resident outcomes. Participate in quality improvement initiatives to ensure high-quality care and regulatory compliance.
Required Qualifications
- 1+ years of experience in resident assessment, nursing, or a related field
- Strong understanding of Medicare and Medicaid regulations, including RAI and MDS
- Excellent communication, interpersonal, and organizational skills
- Ability to work effectively in a fast-paced environment with multiple priorities
- Strong analytical and problem-solving skills
