Job Summary
Connect with eligible Black birthing individuals during prenatal and postpartum visits, providing support to Queens Village programming and addressing social determinants of health. Complete timely follow-up on patient referrals received via Epic and document all interactions in the Electronic Health Record. Participate in daily clinic huddles and interdisciplinary meetings to align patient flow, while building relationships with physicians and staff to normalize Queens Village as part of the care team. Maintain and curate the Queens Village Corner lobby resource station and coordinate onsite community programming in partnership with leadership. Serve exclusively Black birthing individuals through culturally centered community support integrated into the University of Cincinnati Medical Center prenatal workflows.
Required Qualifications
- High school diploma or equivalent
- 2+ years of work experience in a related job discipline
- Experience working within clinical/healthcare settings
- Familiarity with maternal and infant health
- Familiarity with social determinants of health
- Familiarity with health disparities
- Must be able to connect with all eligible Black birthing individuals during prenatal and postpartum care visits
- Must be able to provide support and connection to Queens Village programming by building relationships, maintaining continuous communication throughout pregnancy and the postpartum period
- Must be able to connect patients to internal resources to address Social Determinant of Health needs
- Must be able to receive patient referrals via Epic and complete timely follow-up during onsite days
- Must be able to participate in daily clinic huddles and interdisciplinary meetings to align patient flow and identify engagement opportunities
- Must be able to build relationships with physicians, nurses, social workers, residents, and clinic staff to normalize Queens Village as part of the care team
- Must be able to provide ongoing education and guidance to clinic staff on Queens Village programs and referral processes
- Must be able to document all patient interactions and communications in the Electronic Health Record (EHR)
- Must be able to collect appropriate data for Cradle Cincinnati reporting requirements
- Must be able to document any community resource referrals; record relevant feedback from those resources in the patient EHR to support care team collaboration
- Must be able to maintain and curate the Queens Village Corner in the main lobby, ensuring it is welcoming and stocked with up-to-date resource materials
- Must be able to promote and share information about Queens Village community events with all eligible patients
- Must be able to coordinate onsite Queens Village programming/activities in partnership with UC Center for Women's Health leadership
- Must be available for 4 days onsite at UC Center for Women's Health and 1 remote day per week
- Must be available for weekend shifts
- Must be able to lift 50 lbs
Desired Qualifications
- Current Community Health Worker (CHW) certification in the state of Ohio
- Experience serving, collaborating with, and building relationships with diverse populations within urban communities
