Job Summary
Conduct HEDIS reporting by reviewing medical records to abstract clinical data elements, verify laboratory results, and identify missing measures for quality improvement. Collect medical record data, flag noncompliant charts, and request documentation from outside resources. Manage group and provider-level performance reports, upload HEDIS data to health plans, and engage clinic staff in developing strategies to close quality gaps. Support the Wellness & Preventative Management Program with appointment coordination and care management tasks. This role supports Mount Sinai Medical Center's mission to deliver clinical excellence across South Florida's diverse community.
Required Qualifications
- High School Diploma
Desired Qualifications
- Certified Medical Assistant or Registered Medical Assistant
- Bachelor of Sciences degree or in other related field, foreign medical graduate
- One year of experience in Care Coordination in outpatient setting or HMO, MSO or ACO
