Job Summary
Conduct outbound patient calls to ensure chronic disease medication refills for diabetes, blood pressure, and cholesterol, serving as the liaison between patients, clinics, and pharmacies to close care gaps. Interpret medical history and EMR data to accurately document care activities, perform medication reconciliation, and request refills per policy. Identify patients needing pharmacist referrals or care management, coordinate appointments and screenings, and provide persistent outreach to ensure adherence. Maintain current knowledge of quality measure requirements and assist other quality departments as needed. Requires high school diploma, healthcare setting experience, and proficiency with EHR platforms.
Required Qualifications
- High school diploma or GED equivalent
- Certified Medical Assistant
- Certified Pharmacy Technician
- equivalent experience in a healthcare setting
- Intermediate to advanced level keyboarding skills
- at least 35 WPM

- Secure workspace at home

- Candidate supplied home internet access
- Minimum internet download speed of 100mbps

Desired Qualifications
- 1+ years of experience in a healthcare setting
- Prior experience using an Electronic Health Record (EHR)
- Previous call center or telephonic experience
- Experience with Medicare Advantage plans
