Job Summary
Quality & Population Health Coordinator role supporting quality improvement and population health initiatives at an FQHC. Responsibilities include proactive patient outreach, care gap closure, referral follow-up, panel management support, scheduling, documentation, and data reporting related to HEDIS/GPRA/UDS/HRSA and value-based care measures. Collaborates with clinical and operational teams to improve continuity of care, preventive service completion, chronic disease management outcomes, and patient retention. Performs non-clinical outreach, engagement, scheduling support, and quality workflow coordination; does not provide clinical assessment or independent care management. Requires strong communication, organization, and ability to work with diverse populations; familiarity with ECW EHR and population health concepts preferred. Physical and mental requirements include ability to lift up to 10 pounds, extensive computer use, and maintaining confidentiality. Preference given to American Indian/Alaskan Native applicants; equal opportunity employer.
Required Qualifications
- High School Diploma or GED (equivalent)
- 2-3 years related experience and/or training, or equivalent combination of education and experience
