Quality - Quality and Population Health Coordinator

Quality & Population Health Coordinator role supporting quality improvement and population health initiatives at an FQHC. Responsibilities include proactive patient outreach, care gap closure, referral follow-up, panel management support, scheduling, documentation, and data reporting related to HEDIS/GPRA/UDS/HRSA and value-based care measures. Collaborates with clinical and operational teams to improve continuity of care, preventive service completion, chronic disease management outcomes, and patient retention. Performs non-clinical outreach, engagement, scheduling support, and quality workflow coordination; does not provide clinical assessment or independent care management. Requires strong communication, organization, and ability to work with diverse populations; familiarity with ECW EHR and population health concepts preferred. Physical and mental requirements include ability to lift up to 10 pounds, extensive computer use, and maintaining confidentiality.
