Job Summary
Monitor and report on key clinical quality measures such as cancer screenings and diabetes care. Develop and manage patient registries to identify care gaps, then conduct outreach to support preventive services and chronic disease management. Participate in payer case conferences, prepare performance reports for care teams, and assist with documentation and quality improvement initiatives. Address patient complaints related to care quality as needed. This full-time role includes occasional evenings and offers a comprehensive benefits package. The team collaborates with providers and external partners to drive data-informed improvements and enhance patient outcomes within Callen-Lorde's LGBTQ+ healthcare mission.
Required Qualifications
- High school diploma
- Bachelor's degree preferred
- LPN or experience with medical assisting preferred
- 2-3 years of experience in health care
- Prior experience in a healthcare or social services setting and/or a non-profit organization
- Prior experience working with health plans
- Effective communication skills with patients, colleagues, and other healthcare professionals
- Ability to handle multiple tasks and work under stress in a dynamic environment, managing patient care needs effectively
- Demonstrate proficiency in essential digital tools and healthcare technology including Microsoft Office Suite (Word, Excel, PowerPoint, Outlook), Electronic Health Record (EHR) systems, and other reporting software
- Excel in delivering patient-centered care with a strong focus on customer service, emphasizing empathy, active listening, and responsive communication
- Collaborate effectively within a team environment, supporting colleagues and contributing to shared goals
- Proficiency in more than one language (e.g., Spanish/English)
