Job Summary
Conduct patient outreach and education via telephone, text, and patient portal regarding initial health assessments, continuity of care, and chronic condition management. Coordinate primary care provider assignments using the Four-Cut Method and screen patients for social determinants of health to connect them with community resources and health plan services. Participate in PDSA quality improvement projects, report outcomes, and support data collection for regulatory reporting. Maintain operational efficiency while adhering to organizational quality standards and confidentiality protocols. Requires Medical Assistant certification or Bachelor's degree with 3-4 years of primary care experience.
Required Qualifications
- Medical Assistant certification or program completion
- High school diploma or GED
- 3-4 years of healthcare experience in a primary care setting
- Bachelor's degree
- Exposure to healthcare
- Knowledge of or experience with HEDIS
- Experience with UDS
- Basic Life Support from American Heart Association
- Spanish speaking
- Ability to maintain excellent internal and external customer service
- Ability to maintain the highest degree of confidentiality
- Tact necessary to deal effectively with patients, providers, and employees
- Ability to maintain confidentiality
- Ability to work independently
- Ability to multitask

- Ability to adhere to Clinica Sierra Vista's employee health/immunization requirements or provide a valid exemption request
Desired Qualifications
- Existing employees in this role prior to June 2025 will be grandfathered in
- Medical Assistant certification or program completion WITH a high school diploma or GED and 3-4 years of healthcare experience in a primary care setting
- Bachelor's degree
- Exposure to healthcare preferred
- Knowledge of or experience with HEDIS and UDS preferred
- Basic Life Support from American Heart Association preferred
- Spanish speaking highly preferred
