Job Summary
Conduct Annual Wellness Visits per Medicare guidelines, assessing healthcare, educational, and psychosocial needs to establish realistic treatment plans. Investigate healthcare options and facilitate communication among patients, primary care providers, and the care team to eliminate barriers and coordinate disease management services. Educate patients and families on Care Coordination processes, disease management programs, and outcomes while making referrals as needed. Collaborate with providers to enhance evidence-based clinical guideline adherence, initiate therapies, and coordinate laboratory and diagnostic tests for value-based payment programs. Enter patient data into medical records according to department standards and participate in continuing education. Requires a BSN, New York RN license, and minimum three years of ambulatory experience.
Required Qualifications
- Bachelor of Science in Nursing (BSN) degree
- New York Registered Nurse (RN) License & Registration
- Minimum of 3 years ambulatory experience
- Strong background in ambulatory care prevention, health promotion, condition management, case management and discharge planning
- Mastery of theories, principles and methodologies underlying population health management and clinical preventative services
- Knowledge of contemporary practice transformation models, e.g., PCMH, APC
- Proficient in Microsoft Office (Excel, Power Point, and Microsoft Word), Outlook (email), and internet familiarity
- Knowledge of EMR (EPIC preferred)
