QUALITY AND POPULATION HEALTH MANAGER

Job Summary
Lead quality improvement initiatives across patient safety, population health, and clinical outcomes by overseeing RN case managers, facilitating monthly committee meetings, and coordinating care transitions. Monitor and report quality metrics, conduct random quarterly chart reviews, and collaborate with health plan teams to execute support programs and maintain national accreditations like PCMH. Develop policies, workflows, and staff education forums while analyzing data to align with value-based goals. Manage the department's operating budget and human resources, ensuring compliance with FQHC operations and NCQA standards. Requires five years of experience in quality improvement and case management, plus Nevada RN licensure.
Required Qualifications
- Graduate of an accredited nursing school
- Licensure as a registered nurse in the state of Nevada
- Knowledge of FQHC operations, HRSA requirements, NCQA standards, and UDS reporting measures
- Current CPR or BLS certification
- Active Nevada Drivers License
- Minimum of five years of experience in quality improvement and case management activities
- Working knowledge of Electronic Health Record systems
Desired Qualifications
- Master's degree in nursing
- Working knowledge of successful project management
- Knowledge of the essential functions, practices and procedures of a medical clinic and office
- Working knowledge of reporting and measurement of performance activities
