Job Summary
Support clients from initial contact through therapy, resource linking, and care gap bridging for individuals on the waitlist. Deliver clinical coverage across programs, including crisis intervention, via home visits, community settings, or telehealth. Connect families with vital community resources and collaborate with supervisors to ensure smooth care transitions. Lead outreach and education efforts to promote program access while strengthening relationships with referral sources and partners. Requires a Bachelor's in social work or counseling, one year of post-graduate experience, and bilingual fluency is a plus. Required Qualifications
- Bachelor's degree in social work, counseling, psychology, or a related field
- At least one year of post-graduate counseling experience

- Excellent organizational skills, attention to detail, and the ability to balance multiple responsibilities and meet deadlines

- Comfort providing services in the community (must have reliable transportation)
- Experience supporting youth and families from diverse backgrounds

- Proven ability to build and sustain positive relationships across the community
