Job Summary

Lead admissions and census management by conducting pre-admission screenings, meet-and-greet visits, and determining program compatibility for prospective individuals. Coordinate the full admission process, develop initial Person-Centered Plans, and actively manage referrals to maintain bed occupancy. Serve as the primary interdisciplinary team coordinator by facilitating meetings, interviewing individuals to define goals, and supervising the implementation of personalized care plans. Conduct regular site visits to evaluate service quality, ensure environmental safety, and review internal audits for continuous improvement. Maintain strong communication with families and guardians while ensuring accurate documentation in accordance with Medicaid and regulatory requirements.

Required Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, Special Education, or a related field
  • At least 2 years of experience (in North Carolina)
  • 1 year of experience (in Tennessee) working with individuals with intellectual and developmental disabilities
  • 1 year required if Master's degree is held
  • Demonstrated experience in developing, writing, and implementing Person-Centered Plans
  • Working knowledge of ICF regulations, person-centered planning principles, and applicable Medicaid/HCBS rules
  • Excellent communication, organizational, and documentation skills
  • Valid driver's license, auto insurance, and reliable transportation
  • Must be at least 18 years of age
  • Must be able to regularly lift and carry up to 20 lbs.
  • Must be able to occasionally pull at least 40 lbs.
  • Must be able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties
  • Must be able to use hands and fingers to handle or operate objects, tools, or controls
  • Required to stand and walk for extended periods
  • Must be able to demonstrate proficiency in CPR from floor level
  • Vision requirements include close, distance, and peripheral vision
  • Must be able to talk and hear
  • Complete criminal background Name checked in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements
  • Proof of employment history or references (if required)
  • Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals
