Job Summary
Plan, coordinate, and monitor services to ensure continuity and quality care for adults with mental health needs. Develop and monitor person-centered plans while providing supportive counseling and crisis management. Assist with vocational, housing, and other basic needs, then document services accurately in the information system. Participate in team meetings and clinical training events to maintain standards. Required Qualifications
- Bachelor's degree in a human services field
- Two years of experience with the mental health population
- Non-human services degree with four years of mental health experience
