Job Summary
Develop individualized short-term goals and interventions for Service Recipients receiving Innovations Waiver or State Funded Services. Conduct clinical and employee supervisory visits to ensure care standards and compliance, while providing ongoing development plans and annual performance evaluations for direct care staff. Advocate for recipients in ISP meetings, monitor billing, maintain accurate documentation, and resolve grievances. Represent Abound Health at community events to spread awareness and build connections. This role requires a Bachelor's degree in a related human service field with two years of IDD experience, plus a hybrid schedule.
