Job Summary

The Qualified Mental Health Professional (QMHP-CS) is tasked with providing Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families. Responsibilities include delivering case management and skills-based interventions, documenting services, attending Wraparound Team meetings, and communicating participant progress. The role demands a Bachelor’s or Master’s degree in a human service-related field, experience in home and community services, and requires reliable transportation, a valid driver’s license, and auto insurance.

Required Qualifications

  • Experience in the provision of home and community-based services
  • Reliable transportation
  • Valid driver’s license
  • Current auto insurance coverage

Desired Qualifications

  • CPR/First Aid Certification preferred
  • Behavior Management (MANDT) Certification a plus
  • Bi-Lingual/Spanish Speaking is a plus
  • Strong written and verbal communication skills
  • Basic computer knowledge
