Job Summary
The Qualified Mental Health Professional – Community Services (QMHP-CS) is responsible for providing Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families. The role involves delivering case management and skills-based interventions to enhance the youth’s natural supports, fostering resilience and recovery, while ensuring compliance with relevant regulations. The QMHP-CS must maintain accurate documentation, participate in team meetings, and communicate participant progress effectively. Candidates need a Bachelor's or Master’s degree in a human service field, strong assessment skills, and the ability to work independently and collaboratively.
Required Qualifications
- Bachelor's or Master’s degree in a human service related field
- Strong assessment and diagnostic skills
- Excellent verbal and written communication abilities
- Demonstrated ability to work effectively both independently and as part of a team
- Proficiency with Microsoft Office; Experience working with Electronic Health Record (EHR) Systems
- Availability to work flexible hours, including non-traditional hours, some evenings and weekends
- Experience in the provision of home and community-based services
- Position requires reliable transportation, valid driver’s license, and current auto insurance coverage
Desired Qualifications
- Valid state licensure (LCSW, LPC, LMFT, etc.) as a mental health professional
- Knowledge of evidence-based treatment approaches for various mental health conditions
- CPR/First Aid Certification
- Behavior Management (MANDT) Certification
- Bi-Lingual/Spanish Speaking
