Job Summary
Coordinate discharge processes to ensure successful patient transitions in collaboration with the healthcare team. Conduct assessments on patients and caregivers throughout hospitalization and outpatient follow-up as needed. Identify risk factors for post-discharge undesirable events and apply targeted interventions to mitigate them. Bridge communication between discharge services, outpatient providers, and patients/caregivers to address specific issues and questions. This Night-1 shift role (3:00pm–11:30pm) at MUSC Community Physicians supports the Lexington County Detention Center. Candidates must hold an MSW with South Carolina licensure.
Required Qualifications
- Master's degree in a social work or social welfare program accredited by the Council on Social Work Education
- MSW licensure by the South Carolina Board of Social Work Examiners
- Eligible for Shift Differential
- Night Shift 3:00pm - 11:30pm
