Job Summary
Qualified Health Home Specialists advocate for, facilitate, and coordinate services that a person needs to pursue their goal of recovery. They collaborate with Integrated Care Managers as part of an integrated team to support assessment and planning, comprehensive care management and care coordination, facilitation of Recovery Team Meetings, health promotion and wellness, client and family support, referral to needed services, and where available, referral to Peer Support Services. The role involves partnering with Integrated Care Managers to support assessment/planning, care coordination, and referral/monitoring to ensure integrated care aligned with the client's recovery vision.
