Job Summary
Clinical Case Manager (QMHS) responsible for providing case management services, coordinating treatment and discharge planning, developing individualized treatment plans with the treatment team, and maintaining communications with referral sources and other services to support clients' recovery.
Required Qualifications
- High School Diploma required.
- One year of experience in a substance use and/or mental health treatment environment required.
