Job Summary
Coordinate discharge processes and psychosocial assessments for patients in an acute care psychiatric setting, ensuring successful transitions through collaboration with the healthcare team. Conduct active treatment including group therapy, manage case management for a complex caseload, and identify risk factors to apply targeted discharge interventions. Bridge communication between discharge providers, patients, and caregivers to address post-hospitalization issues. Perform this work during 6-hour shifts on Saturdays and Sundays. Requires Master's degree in social work, LMSW/LISW licensure, and certification in Safety Care and Basic Life Support.
Required Qualifications
- Master's degree in a social work or social welfare program accredited by the Council on Social Work Education
- One years post graduate professional social work experience
- LMSW/LISW licensure by the South Carolina Board of Social Work Examiners
- Certified in Safety Care and Basic Life Support within the first 90 days of employment and annually thereafter
