Job Summary
Conduct jail report generation, mental health assessments, and face-to-face crisis interventions for individuals with chronic mental illness within Collin County Detention Facility. Perform crisis follow-up and relapse prevention services, coordinate case management, and organize recovery plans adhering to federal, state, and local guidelines. Transport individuals in a center vehicle and provide services at community-based locations including schools and hospitals. Complete detailed documentation concurrently with services, ensuring signed charts by shift end and maintaining 90% chart review scores of 80 or higher. Execute assigned follow-ups at 24-hour, 1-7 day, 30, 60, and 90-day intervals while attending weekly treatment team meetings. Work 36 hours weekly as a .9 FTE with potential holiday coverage, utilizing de-escalation techniques for potentially dangerous situations.
Required Qualifications
- Bachelor's degree from an accredited college or university with major course work in a human services field
- Two (2) years prior experience in a mental health field
- Reliable transportation
- Valid driver's license
- Knowledge of community resources
- Knowledge of case management principles, objectives, standards, and methods
- Knowledge of program policies and procedures
- Ability to complete detailed, concise progress notes on all services provided during the sessions
- Knowledge and experience in providing rehabilitative services to individuals with mental illness
- Ability to assess individual's needs
- Ability to coordinate services
- Ability to communicate effectively
- Ability to train others
- Knowledge of the social services system
- Ability to connect individuals with appropriate services
- Ability to write clearly and professionally
- Ability to think clearly and logically
- Ability to utilize de-escalation techniques

- Services will be provided 24 hours per day, 365 days per year, including holidays
- Must be available for holidays if the holiday falls on their scheduled workday
- Must notify supervisor within 4 hours of regularly scheduled work time if using Paid Time Off
- Position is a .9 full time equivalent, requiring 36 hours of work per week
- Hiring for shift- THURS, FRI, SAT 6AM-6PM; with 4 hours on WED (between 2AM-6PM) to complete 40 hour commitment
