Job Summary
Conduct ongoing monitoring, support, and behavioral health services for children, youth, and families with emotional trauma through weekly skills training and case management. Develop individualized Service/Treatment Plans and Crisis Plans within 10 days of intake and reassess client needs using CANS/ANSA assessments every 90 days. Facilitate team meetings, modify treatment plans based on progress, and provide crisis support via phone or in-person visits. Deliver 20-25+ billable hours weekly across 25-35 clients by meeting face-to-face in homes, schools, and communities while entering all data into the electronic record system within 24 hours. Maintain accurate documentation of all services and collaborate with the clinical team to build autonomy and safety for the child, youth, and family.
Required Qualifications
- Bachelor's degree in human services or other related discipline
- Proof of valid Texas Driver's License (Type C)
- At least three years of driving experience
- Access to reliable transportation
- Proof of valid/current auto insurance
- Cleared motor vehicle driving record
- Three professional employment references
- Cleared criminal background check and signed statement regarding felony indictments/convictions
- Cleared TB test results (current within 12 months prior to employment)
- Cleared pre-employment drug screen
- Working cellular telephone
- Ability to appropriately accept feedback through the supervision process - thus displaying the willingness to learn, grow, and improve
- Ability to work efficiently and effectively both individually and as part of a team
- Ability to work with children and families to deliver skills through approved curriculum
- Effective oral and written communication skills
- Well-organized and adaptable
