Job Summary
Provide case management, skills training, medication training and support, crisis interventions and psychosocial rehabilitation services with a trauma-informed and person-centered approach for individuals with behavioral health needs. Travel to homes, schools, and community settings is required, with after-hours availability to meet program needs. Maintain 80+ hours per month of engagement documented in the electronic health record, and ensure fidelity standards through audits. Build relationships with community agencies and stakeholders, promoting autonomy and navigating access to medical, social, vocational, financial, educational, and other services. Demonstrate strong writing and communication skills, assess and de-escalate crisis situations, and collaborate independently or with teams. Bilingual capability with translation support if stipend is received.
Required Qualifications
- Bachelor’s degree in psychology, social work, or related field from a four-year college or university
- At least two years of experience in working with the community preferred (i.e., social services, recovery, customer service, sales)
- Bilingual (Spanish/English) preferred
- Must have reliable transportation and a valid driver license
