Job Summary
Conduct intensive evaluation, consultation, and intervention with youth and families participating in wraparound services or YES waiver within the Heart of Texas Behavioral Health Network catchment area. Provide case management, skills training, and psychosocial programming while performing assessments, treatment recommendations, and referrals to social welfare agencies. Create a Child and Family team for each caseload, develop crisis/safety plans, and lead Family Team meetings. Establish a plan of care for every individual family and maintain documentation to support program implementation. Requires a valid Texas driver's license, current liability insurance, and full LPHA licensure or candidate status. Full-time role with local and regional travel.
Required Qualifications
- Must have a current valid Texas driver's license
- Must have current and continued proof of liability insurance
- Must be fully licensed or licensure candidate for LPHA
- A bachelor's or master's degree in psychology, social work or related field from an accredited college or university
- Ability to lift up to 15lbs
- Ability to travel locally 75% and regionally 25%
- Must complete Wraparound training
