Job Summary
ACT case manager responsibilities include conducting assessments, recovery planning, and coordinating crisis services and community-based behavioral health support for adults; role involves referrals to housing, supported employment, and outpatient services, responding to occasional after-hours needs, and collaborating with LifePath Systems and community partners to promote patient independence and reduce hospitalizations; requires strong organization, communication, trauma-informed care, and ability to navigate social services systems; bilingual ability (English/Spanish) preferred, with Bachelor’s degree required and Master’s degree preferred.
Required Qualifications
- Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
- Knowledge of Trauma-Informed Care Practices required.
