Job Summary
Provide routine case management, skills training, medication training, crisis interventions, and psychosocial rehabilitation services within a trauma-informed, person-driven care model. Maximize resources to enhance Lifepath Systems operations while supporting client strengths and navigating medical, vocational, financial, and educational services. Ensure state standards and Lifepath Systems policies are met through timely documentation and independent, collaborative case management. Requires a Bachelor's degree in psychology or social work, two years of community experience, and bilingual Spanish/English proficiency. This role supports individuals with mental illness and contributes to measurable improvements in service delivery and community outcomes.
Required Qualifications
- Bachelor's degree in psychology, social work, or related field from a four-year college or university
- Ability to assess individual needs
- Ability to coordinate services
- Ability to communicate effectively
- Ability to write clearly and professionally
- Ability to think clearly and logically in order to deal with potentially dangerous and/or emotional situations
Desired Qualifications
- At least two years of experience in working with the community, preferred
- Bilingual (Spanish/English), preferred
