Job Summary
Supervise the development and implementation of each client's Individual Habilitation plan by integrating services following their individual service plan. Oversee the continuity and quality of client services, including facilitating participation in community outings and recreational activities. Assist in direct support staff orientation, training, and mentoring while monitoring ISP implementation through monthly summaries and plan revisions. Promote individual rights, healthy lifestyle choices, and access to appropriate resources for educational and wellness activities. Participate in long and short-term program committees, human rights reviews, and interdisciplinary team meetings. Maintain open communication with management, consultants, families, and school personnel. Adhere to all agency policies and procedures.
Required Qualifications
- Bachelor's degree in a field related to human services or developmental disabilities
- Minimum of 2 years of direct experience in the care and training of persons with developmental disabilities
