Job Summary
Build relationships with clients and families to ensure services reflect unique needs, coordinate service agreements and authorizations, and develop implementation strategies with support teams. Maintain accurate electronic records in Quantum, provide advocacy during crises, and partner with community resources for holistic care. Travel throughout Cook County for client visits, approximately 70% in the community, requiring a valid driver's license. This role supports adults with developmental disabilities in living full, meaningful lives within a collaborative, mission-driven team that values inclusion and offers ongoing training opportunities.
Required Qualifications
- Must be QIDP-eligible upon hire
- At least 1 year working with adults with developmental disabilities
- Bachelor's degree in Human services field, including, but not limited to Sociology, Special Education, Rehabilitation Counseling or Psychology, Recreation or in a specialty area such as Art, Dance, Music or Physical Therapies
- Valid driver's license and reliable vehicle
- Flexibility and willingness to travel locally (mileage and gas reimbursed)
- Must be able to stand, walk, and sit
- Must be able to see, hear, and listen
- Must have clear speech
- Must have dexterity and use of hands and fingers
