Job Summary

Conduct case management, skills training, medication support, crisis interventions, and psychosocial rehabilitation services for individuals with behavioral health needs using a trauma-informed, person-centered approach. Gather information and establish eligibility for community resources while frequently traveling to homes, schools, and community settings. Provide person-directed case management to assist with navigating medical, social, vocational, financial, and educational services. Document engagement in the electronic health record at a minimum of 80 hours per month and ensure documentation meets fidelity standards within two business days. Perform internal and external fidelity audits, maintain equipment, and complete required trainings. Build positive relationships with community agencies and stakeholders while assessing and de-escalating behavioral health crisis situations. Promote choice and autonomy to improve access to social determinants of health.

Required Qualifications

  • Bachelor's degree in psychology, social work, or related field from a four-year college or university
  • Must have reliable transportation and a valid driver license
  • Knowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illness
  • Ability to assess individual needs, to coordinate services, and to communicate effectively
  • Knowledge of the social services system and the ability to connect individuals with appropriate services
  • Ability to write clearly and professionally
  • Ability to think clearly and logically to deal with potentially dangerous and/or emotional situations
  • Frequent travel to individual's homes, schools, and community settings
  • Ability to work outside of normal business hours
  • Integrity to ensure adherence to Health and Human Services Commission, Texas Administrative Code, contracts, grants, and Lifepath Systems policies and procedures
  • Ability to assess and de-escalate behavioral health crisis situations in calm and respectful manner

Desired Qualifications

  • At least two years of experience in working with the community, preferred (i.e., social services, recovery, customer service, sales)
  • Bilingual (Spanish/English), preferred
