Pulmonary APP

Pulmonary Nurse Practitioner position providing personalized, compassionate medical care to patients primarily with COPD and other pulmonary conditions. Responsibilities include patient health assessment, care coordination, medication management, ordering tests, prescribing medications, documenting comprehensive medical records, and collaborating with pulmonology specialists, field-based nurses, and care teams. The role conducts assessments both in patients' homes and in virtual settings, educates patients and families about benefits and programs, develops treatment plans, and supports follow-up to improve quality of life and reduce avoidable hospitalizations. Requires autonomy, strong communication, teamwork, and a focus on evidence-based care; remote in-state work with periodic training outside of the employed state. Positions emphasize adherence to quality standards, documentation, and participation in integrated care team meetings.
