Job Summary
Care Coordinator to manage client caseloads and coordinate quality home care services across Southern Colorado. Responsibilities include assessments, client visits, caregiver scheduling and coverage, documentation, and proactive communication with families, providers, and partner agencies to ensure compassionate, effective care. Works from home with occasional in-office meetings, visits to client homes, and traveling throughout the field (travel up to 50%). Strong communication, organizational, and problem-solving skills are essential; healthcare, case management, or caregiving experience preferred.
