Public Health Nurse ***Independent Contractor***

Job Summary
Conduct comprehensive initial assessments for families newly enrolled in the Non-Medical Family Home Visiting program, including up to 35 visits per contract year. Administer validated screening tools for depression, intimate partner violence, child development, and parent-child interactions. Document TANF eligibility factors and family strengths across health, housing, food security, employment, and social support domains. Initiate immediate safety planning for urgent concerns and submit documentation to support case records and eligibility files. Review findings with families to inform individualized service plans. Requires an active Minnesota RN license, 2+ years of public health or home visiting experience, and valid driver's license.
Required Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Minnesota
- Minimum 2+ years of experience in public health nursing, maternal/child health, home visiting, or a related clinical setting
- Valid driver's license and access to reliable transportation
- Must pass a criminal background check
- Must carry professional liability insurance or provide proof of coverage as required by contract
Desired Qualifications
- Public health nursing experience or certification
- Experience administering standardized screening tools (e.g., EPDS/PHQ-9, HITS, ASQ)
- Experience working with low-income families and communities of color
