Job Summary
Physical Therapy Assistant (PTA) in Home Health provides direct PT services under a licensed Physical Therapist, delivering one-on-one, patient-centered care in patients' homes. Responsibilities include implementing and progressing individualized care plans, guiding therapeutic exercises, gait training, balance and functional mobility work, assisting with transfers and activities of daily living, utilizing modalities (heat, cold, ultrasound, electrical stimulation) as appropriate, educating patients and caregivers, documenting progress in the EMR, and collaborating with the PT team and interdisciplinary care staff. The role emphasizes hands-on rehabilitation, autonomy with team support, and ongoing training within a growing home health organization.
Required Qualifications
- Graduate of an accredited Physical Therapy Assistant (PTA) program
- Current PTA license or state certification as required by state law
- Current CPR certification
- Valid driver’s license and reliable transportation
- 1+ year of experience preferred (home health experience a plus)
- Ability to collaborate with therapists, nurses, and care teams
