PTA - Physical Therapy Assistant - Home Health Care Visits

Administer physical therapy to patients in their homes under the direction of a Registered Physical Therapist, adhering to physician orders and established agency policies. Improve or minimize residual physical disabilities and return individuals to optimum productivity by performing skilled procedures, consulting on treatment changes, and instructing patients and families on in-home activities. Prepare medical records, progress notes, and update care plans for each visit while participating in patient care planning.
