Job Summary
Conduct home-based physical therapy treatment, instruct patients on active and passive exercises, and guide families in functional mobility, gait training, and transfer safety. Perform procedures only within your qualified scope while strictly adhering to the supervising PT's established plan of care. Observe patient responses to treatment, record progress, and report condition changes to the supervising PT, nurse supervisor, or physician. Coordinate care with the broader healthcare team and make recommendations for plan adjustments as needed. Maintain a current driver's license and vehicle insurance, and be available to work a weekend rotation.
Required Qualifications
- Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current driver's license and vehicle insurance
- Access to a dependable vehicle
- Public transportation
- Available to work a weekend rotation
- Ability to pass a drug test
Desired Qualifications
- Current CPR certification
- Ability to complete CPR certification within 90 days of hire
- Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility
