Provide individualized physical therapy treatments according to the established plan of care under the direction of a licensed PT. Assist patients with therapeutic exercises, gait training, balance activities, transfer training, and mobility techniques to improve mobility and reduce pain. Monitor patient progress, communicate changes in condition to the supervising PT and care team, and educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques. Complete accurate and timely clinical documentation while collaborating with physicians, nurses, occupational therapists, and speech-language therapists to ensure coordinated patient care. Maintain compliance with agency policies and state and federal home health regulations.
