Deliver physical therapy interventions and therapeutic activities to designated patients under the supervision of a Physical Therapist, including positioning, transferring, and lifting patients while instructing family members. Manage assigned caseloads by following established plans of care, modifying procedures as needed, and documenting all services per VNS Health policy. Assist patients with durable medical equipment, transport required technology devices to and from patient homes and care facilities, and monitor progress through direct observation. Comply with NY State Education law requirements, including co-visits with a Physical Therapist on all initial visits and re-assessments every six visits or 30 days. Participate in interdisciplinary team meetings, in-service training, and caseload reviews while ensuring adherence to agency standards and infection control protocols.
