Deliver physical therapy interventions to designated patients under the supervision of a Physical Therapist, including positioning, transferring, and lifting patients of varying sizes while instructing family members. Manage assigned caseloads by following established plans of care, modifying procedures as needed, and documenting all services per VNS Health policy. Assist patients with durable medical equipment and travel to home environments or care facilities using approved transportation. Monitor patient progress through observations and follow up with the supervising therapist, ensuring compliance with NY State Education law and professional scope of practice. Participate in interdisciplinary team meetings, in-service training, and caseload reviews.
