Job Summary
Perform treatment, provide exercise instruction, and deliver patient education aimed at improving well-being while following the established plan of care. Instruct patients and caregivers in active and passive exercise, gait training, and functional mobility. Observe, record, and report patient responses to treatment and condition changes to supervising PTs, nurses, and physicians. Coordinate care with the broader healthcare team and make recommendations for plan adjustments. Follow all state-specific laws governing physical therapy in home care and perform only procedures within your qualified scope. Work a weekend rotation in any home situation regardless of client demographics.
Required Qualifications
- Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
- Available to work a weekend rotation
- Candidates are required to pass a drug test before beginning employment

- Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility
