Job Summary
Design and implement individualized treatment plans addressing fine and gross motor skills, visual-motor integration, mobility, balance, strength, and conditioning for pediatric and adult clients with special needs in Assisted Living Facilities. Support fall prevention initiatives and health and wellness programs while providing effective communication with clients, caregivers, and interdisciplinary team members. Educate and train caregivers, including Home Health Aides, to ensure consistency and skill carryover. Complete clinical documentation accurately within required timeframes and travel to multiple facilities across the five boroughs of New York City and Long Island. Compensation is $85.00 per visit for fee-for-service work.
Required Qualifications
- Valid New York State Physical Therapist (PT) License
- Strong knowledge of anatomy, physiology, and geriatrics
Desired Qualifications
- Experience in clinics, skilled nursing facilities, assisted living, or home health settings
